Tunstall launches next generation Integrated Care Platform an industry first for transforming community care for patients with long term conditions

New concept in telehealthcare will provide integrated, managed and scalable patient centred care to unlock efficiencies and improve patient experience.

Tunstall Healthcare Group, the leading provider of telecare and telehealth solutions, is today launching icp integrated care platformTM, its next generation suite of telehealth solutions at the Managing Long Term Conditions conference in Harrogate.

icp supports integrated, fully managed and patient-centred care that is the key to unlocking the deployment of telehealthcare in a scalable and holistic way.  This enables remote health monitoring that can be tailored specifically to patients' needs to help reduce avoidable hospital admissions, improve quality of life, and help to support people with long-term conditions in their homes for longer.

Tunstall's icp is a suite of solutions that enable key stakeholders to work beyond traditional organisational boundaries. These integrated solutions enable delivery of care that can manage patients seamlessly and consistently through a single user interface and combine care team resources to ensure optimum use of community healthcare resources.

icp supports the management of remotely delivered services, capturing patient information remotely and securely via the mymedicTM product range. It supports reliable and consistent referral of patients to telehealthcare services, linked to risk-based patient identification.

mymedic communicates securely with icp triagemanagerTM, the software platform for use with mymedic, to enable regular interaction with healthcare professionals. icp enables tiered triage of patient information, allowing data to be filtered and validated via icp triagemanager.

Working in partnership with primary care and social care organisations to deliver multiple telehealth services across the UK has given Tunstall a clear understanding of the requirements to enable large-scale telehealth deployments which transform care in the community.
